In most states, yes. Parents, legal guardians, adult children, spouses, siblings, and legal representatives are usually authorized. The form asks your relationship to the person on the record and tells you what proof your state needs.
The full name on the certificate, date and city of birth, mother's maiden name, father's full name, and a photo of your government-issued ID. That's usually it. If your state needs anything else, the form will ask.
